Discharge
The placing authority is required to give 28 days notice of ending a young person's placement with Southern Care
Discharge Details
  • Discharge will take place following consultation with the authority responsible for the young person and others relevant to decision making
  • Where possible Southern Care would prefer discharge to be discussed at a review to confirm arrangements in the young person's care plan
  • The young person will be prepared for moving on using this as a positive stepping stone to his or her future
  • For those young people moving to live independently Southern Care can provide an after care service in consultation with the authority responsible for the young person
  • A comprehensive independent living and pathway plans are provided to all young people moving to live independently
  • In circumstances where discharge is unplanned, if appropriate and in consultation with the registered manager and social worker, the key worker will write a letter to the young person. The letter will include positive aspects of the young person's stay with Southern Care and a copy will be placed on file
  • When discharge has been unplanned a placement breakdown meeting will be held with the Registered Manager and the staff involved with the young person. A record of the meeting will be filed and used for monitoring and future assessment of our service
  • All files relating to the young person leaving care will be checked and signed by the home's Registered Manager before being sent to the head office for archiving.
  • On discharge a checklist of belongings, medical appointments etc. will be completed and forwarded to the young person's new placement
  • Discharge of a young person for whatever reason will not take place without prior consultation with the placing authority
  • When a placement has broken down or other residents are placed at risk from the actions of a young person, it is expected that the placing authority make other more suitable arrangements as soon as possible
