Planned Admission
Every endeavour will be made to encourage the young person to attend his or her planning meeting/review or a representative will be offered to relay his or her wishes and feelings
The planning meeting will confirm arrangements with and services requested from Southern Care and the referring agency and to formulate a care plan to meet the young person's needs. Precise contact arrangements will need to be made at the initial planning meeting
It is requested that a senior manager from the referring agency attends the initial planning meeting and subsequent Statutory Reviews

Process
- After initial contact has been made the placing authority is requested to forward a completed referral form to the placements manager at Southern Care.
- The placing authority will be asked to provide a brief care history and outline the details of the service requested at this point.
- Request for placement forms and placement agreements will need to be authorised and signed by the placing authority's Placement Manager or equivalent
- All relevant Looked After Children documentation will be requested on the admission of the young person into our care, or as soon after in the case of an emergency placement.
- A planning meeting will be arranged within seven days of the young person's admission. The placing authority should ensure details of the placement and invitations to the meeting are given to the young person's parents if appropriate and relevant others
