Unplanned Admission
If a bed is available Southern Care may be able to offer it for an emergency placement providing the following applies;

Process
- The decision to offer the bed is made in consultation with the General Manager, Placements Manager and Registered Manager of the home.
- All available information on the young person is sought
- Admission will not be to the detriment of other residents or place them at risk
- The young person meets the home's criteria
- It is made clear that there is a review of the placement within 72 hours of admission. A date and time will be arranged with the placing authority at the point of admission
Admission Process
- After initial contact has been made, the referring authority is requested to Fax: relevant information to the home offering accommodation
- Request for placement forms will need to be signed by the placements manager or equivalent out-of-hours manager
- If it is not reasonably possible to produce referral information in urgent circumstances, confirmation may be given verbally with an expectation of written confirmation within two working days
- If a request for placement is out of normal working hours the local authority out-of-hours team will provide basic essential information on the young person
- It is an expectation that a care planning meeting will take place within 72 hours of a young person being placed in an emergency. The date and time will be agreed on the admission of the young person
